All students interested in participating in Sports/Intramural activities MUST submit the following forms to the school nurse:

• Preparticipation Physical Evaluation Form

  The physical must have been completed within 365 days of the first practice according to NJ State Rules.

• Health History Update

  An updated Health History Form must be completed before each season (Fall, Winter, Spring) or if a student switches sports.

  All forms must be reviewed by the school physician for medical clearance. Adherence to the due date is mandatory. Missed deadlines will result in ineligibility for sports participation.
